Cancellation Policy

I understand that unanticipated events happen occasionally in everyone’s life. In my desire to be effective and fair to all clients, the following policy will be honored.
Failure to cancel at least 36 hours before the start of the appointment time will result in a charge for 100% of the scheduled appointment price.
Ideally, you’ll give me closer to 24-48 hours notice when possible. I trust your judgment and consideration. Should I need to apply the cancellation policy and you have not paid in advance, the card on file will be charged the full amount for the missed appointment.

TELEPHONE ACCESSIBILITY

If you need to contact me between sessions, please leave a message on my voicemail. I am often not immediately available; however, I will attempt to return your call within 72 hours. In the event that you are out of town, sick or need additional support, phone sessions are available. If a true emergency situation arises, please call 911 or any local emergency room.

SOCIAL MEDIA AND TELECOMMUNICATION

Due to the importance of your confidentiality and the importance of minimizing dual relationships, I do not accept friend or contact requests from current or former clients on any social networking site (Facebook, LinkedIn, etc). I believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship. If you have questions about this, please bring them up when we meet and we can talk more about it.

ELECTRONIC COMMUNICATION

I cannot ensure the confidentiality of any form of communication through electronic media, including text messages. If you prefer to communicate via email or text messaging for issues regarding scheduling or cancellations, I will do so. While I may try to return messages in a timely manner, I cannot guarantee immediate response and request that you do not use these methods of communication to discuss therapeutic content and/or request assistance for emergencies. Services by electronic means, including but not limited to telephone communication, the Internet, facsimile machines, and email is considered telemedicine by the State of Illinois. Under the Illinois Telemedicine Act of 1996, telemedicine is broadly defined as the use of information technology to deliver medical services and information from one location to another. If you and your therapist chose to use information technology for some or all of your treatment, you need to understand that:

You retain the option to withhold or withdraw consent at any time without affecting the right to future care or treatment or risking the loss or withdrawal of any program benefits to which you would otherwise be entitled.


All existing confidentiality protections are equally applicable.

Your access to all medical information transmitted during a telemedicine consultation is guaranteed, and copies of this information are available for a reasonable fee.

Dissemination of any of your identifiable images or information from the telemedicine interaction to researchers or other entities shall not occur without your consent.

MINORS

If you are a minor, your parents must be present in the treatment room with you during a massage or bodywork session. Parents may be legally entitled to some information about your therapy (including coaching, massage, bodywork, and hypnosis). I will discuss with you and your parents what information is appropriate for them to receive and which issues are more appropriately kept confidential.

TERMINATION

Ending relationships can be difficult. Therefore, it is important to have a termination process in order to achieve some closure. The appropriate length of the termination depends on the length and intensity of the treatment. I may terminate treatment after appropriate discussion with you and a termination process if I determine that the therapy or coaching being provided is not being effectively used or if you are in default on payment. I will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose of terminating. If therapy is terminated for any reason or you request another therapist, I will provide you with a list of qualified practitioners to treat you. You may also choose someone on your own or from another referral source.

Should you fail to schedule an appointment for three consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.

Membership:

This is a month to month agreement and can be canceled with written notice no less than 30 days prior to the final withdrawal date (ex: Cancellation on March 15th will result in an April 1st final draft). Memberships expire 30 days from date of purchase. Elevate Healing Arts, PLLC will charge the account that is provided upon registration. I understand that authority will remain in effect until I cancel my monthly membership in writing. In the event of a failed automatic payment, Elevate Healing Arts, PLLC has the right to terminate my membership or package and forfeit all unused sessions. I also understand that in the event my membership is canceled for any reason, I also forfeit my locked in rate. If I re-join in the future, I will rejoin at the current office rate.

COVID-19 Safety Precautions:

An air purifier turned on 20 minutes before the first session of the day and remains on for the duration of the day.

Singeli will be wearing an N95 mask for every session.

The facility has updated their HVAC system

Social distancing and hygienic/ sanitary precautions remain in place throughout our facility

All high touch surfaces are sanitized with a CDC recommended sanitizer frequently throughout the day

Elevate Healing Arts is a cashless business. All payments will be made electronically unless otherwise agreed upon. 

In the event that I (Singeli) suspect I have been exposed to COVID-19, I will cancel all appointments and self-isolate for the remainder of the day until I can obtain testing and have been confirmed negative.

In the event that I test positive for COVID-19, I will inform all clients who may have been potentially exposed and self-isolate until I can be cleared of infection.

In the event that you suspect you have been exposed to COVID-19, you will call Elevate Healing Arts  at (224) 572-9704 to let Singeli know you have been exposed to COVID-19. There will be no penalties and the sessions can be rescheduled anytime after 7 days of the exposure.

PLEASE NOTE IN ORDER TO RECEIVE SERVICES YOU MUST BE VACCINATED (INCLUDING AT LEAST ONE BOOSTER); this includes receiving at least 1 vaccination anytime between 2020-present. If you have been vaccinated anytime between 2020-present you may proceed in the booking process. 

If available, I will upload a copy of my COVID-19 vaccination card to my client portal on Acuity prior to my first session.
