Locked-in Rate: Your monthly rate is locked in. If our prices increase, you'll stay at your current, lower rate as long as your membership is active.

Membership Policies:
You are agreeing to get either one or multiple monthly massage(s), which will be paid for at the time of purchase or if doing a monthly membership via an automatic draft using the information provided through your client portal on Acuity Scheduling. In return, ELEVATE HEALING ARTS, PLLC guarantees to lock in your current membership rate for the following 12 months beginning when you sign up for the membership.
If purchasing a package, all sessions expire within 60 days unless otherwise agreed upon. Any unused sessions DO NOT rollover unless otherwise agreed upon. You forfeit any unused sessions within the 60 days of purchase. After 60 days, the current package will expire and you will need to purchase a new one in order to receive the special pricing.
MEMBERSHIP:
This is a month to month agreement and can be canceled with written notice no less than 30 days prior to the final withdrawal date (ex: Cancellation on March 15th will result in an April 1st final draft). Memberships expire 30 days from date of purchase.

BY SIGNING UP FOR THE MEMBERSHIP, I AGREE THAT I HAVE READ AND UNDERSTAND THE FOLLOWING
I agree to the membership program and packages terms and conditions. I authorize LEVATE HEALING ARTS, PLLC to charge the account I have provided above. I understand that authority will remain in effect until I cancel my monthly membership in writing. In the event of a failed automatic payment, ELEVATE HEALING ARTS, PLLC has the right to terminate my membership or package and forfeit all unused sessions. I also understand that in the event my membership is canceled for any reason, I also forfeit my locked in rate. If I re-join in the future, I will rejoin at the current office rate.
